The form asks for your name, date of birth, smoker status, telephone number and email address so FJJT can contact you about your enquiry. For Income Protection we also ask for your occupation and annual salary. For Accidental Cover we also ask which sport or hazardous activity you want to protect against. For Private Medical we ask whether you already have PMI and, if so, when it renews.
